SCHEDULE OF INITIAL EVALUATION:
January 25, 2010 to Febuary 5, 2010
Monday to Friday, 1:00 to 8:00 pm

PLEASE BE READY FOR THE FF:
   - Personal Copy of Curriculum sheet and copy of student's scholastic permanent record.
   - Registration card together with classcard, completion forms, certification of grades for cross-enrolled subjects from 1st yr 1st semester of submission to the university to the 2nd semester of SY 2009-2010.
   - Present Registration Card for the 2ND semester sy 2009-2010.
   - Approval of credited subjects for transferees signed by the Director.
   - Entrance credentials particularly Form 137-A.
