REQUIREMENTS:
   - Mongol Pencil #2 and black ballpen.
   - Php500.00 examination fee.
   - Certification that the application is currently enrolled in his/her last year in High School (4th yr/5th yr) duly certified by the High School Principal and/or Registrar, with school dry seal.
   - Certified true copy of F138 with school dry seal (for Hogh School Graduates).

QUALIFIED TO APPLY ARE:
- Graduating high school student of the current year.
- High school graduates who have not enrolled in any technical/diploma/degree program immediately after high school.
